10/5/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Josiah was focused during session as evidenced by him openly responding to QMHP-C. Josiah was well dressed and groomed during the session. Josiah was able to cooperate and communicate well during the session. Josiah was in a pleasant mood today and presented good behaviors, as evidence of him smiling and greeting QMHP-C today for the session. Josiah was excited to express his feelings and share his favorite thing he loves about having counseling as a way to help him get through challenging situations and work on his attitude. Behavior: Josiah displayed positive and cooperative behaviors throughout the session and understands how his negative behaviors will cause him to get in trouble and his games taken away from him if he continues to present those negative behaviors. Intervention: QMHP-C engaged in conversation with Josiah about his day. QMHP-C administered psycho educational activity with Josiah and assisted when needed. QMHP-C provided new skills and strategies to help reduce impulsive and hyperactive decisions. QMHP-C taught Josiah effective communication and assertiveness skills to help him successfully resist negative peer pressure. Examples of assertive, effective communication were provided (e.g. I have to leave now”; “I can’t afford to get into any more trouble”). Positive feedback was provided to the Josiah as he displayed a clear understanding to how to use effective communication and assertiveness skills. QMHP-C reinforced Josiah’s positive behaviors by modeling positive behaviors. QMHP-C encouraged Josiah to maintain his behavior and told Josiah that Intervention: QMHP-C was proud of him. QMHP-C offered simple praises to Josiah for exhibiting the appropriate behaviors while in the home. QMHP-C assisted Josiah in monitoring his own behaviors in efforts to promote exercising self-control in focused areas such as respecting boundaries with others. QMHP-C modeled proper positive behavioral expectations for the Josiah. QMHP-C engaged in conversation with the mother to inquire about any changes or new behaviors she notices regarding Josiah not meeting his objectives or disregarding his treatment plan. Although Josiah needs improvement, QMHP-C commended Josiah on his efforts to maintain his ISP objectives throughout the day. QMHP-C continued to establish a positive supportive relationship with Josiah. QMHP-C offered simple suggestions to the Josiah for behaving appropriately in the community and encouraged Josiah to practice the positive behaviors. Response: Josiah communicated thoughts and feelings well during session. Josiah made constant direct eye contact with authority figures during outing. Josiah was pleased to be praised for his behaviors. Josiah stated that he felt he was smart and kind to his family. Josiah communicated that it is often hard to remain positive at all times about himself when he doesn’t always have the best things or necessities that he needs. Josiah discussed numerous situations where he had to adapt even when he didn’t want to. Josiah expressed that he tries to think highly of himself. Josiah communicated that he feels that he will be better behaved, due to having a woman as a counselor. Josiah followed all directions during the session. Response: Josiah acknowledges expectations and reviews goals and objectives with counselor and parent daily. Josiah was acceptive of prompts given by mom and counselor. Josiah agreed to listen to the adult and follow all rules no matter if he sees someone else breaking them. Josiah is reminded to tell an adult if someone is interrupting his learning in anyway. Josiah was doing a good job ignoring distractions and maintaining appropriate behavior in the session. Josiah incorporated numerous strategies that he learned from his previous sessions that he felt worked best for him with meeting his objectives.
